1. You must fill out and return the La Quinta Lacrosse Medical Information Sheet and Waiver(s) available on the website homepage and pay your dues. You must fill out a separate waiver for each season, fall and regular.
YOU WILL NOT SET FOOT ON THE FIELD UNTIL ALL ARE TURNED IN. NO WAIVER, NO MED CARD, NO MONEY, NO PLAY! NO EXCEPTIONS AND NO DEALS!
Fall Ball dues are cost for tourney entry and new jersey.
Regular Season dues are $500 minus any donations you solicit.

2. You must register at www.uslacrosse.org as a high school player and send member number to Coach Baker. Once you get it, it is good for a whole year.
3. You must get pads ASAP. You can get them from EBay, buy used equipment from returning players on the team, any online dealers on the web, or from Sling It Lacrosse- buy online. An excellent beginners package is offered for $199.99. The kit comes with a helmet as well, so be sure to mark on the order that you need a white shell, silver visor, and maroon chin piece to match the team colors.
4. You must be a part of our fundraising! Please be sure to do your part and sell, sell, sell!
6. If you are going to buy your team helmet now, you MUST get a Cascade CPX custom or Cascade CLH2 with a white shell, silver visor, and maroon chin piece. Any variation in the helmets will result in an inability to play. This is a new US Lacrosse rule. All active team players must be equipped with identical helmets or the team will be disqualified.
